Americans for the Arts 2022 National Arts Awards
Assumption of Risk and Waiver of Liability

I, on behalf of myself, my family, and any individuals traveling or staying with me, waive all claims against, and release from all liability, Americans for the Arts and its management, Board of Directors, volunteers, sponsors, vendors, staff, and agents, related to or arising from injury, sickness or death from contraction or spread of COVID-19 or other communicable disease and illness due to travel to or attendance at the 2022 National Arts Awards, even if arising from the negligence (but not intentional wrongdoing) of any such persons.

I understand that traveling and gathering in-person involve risk of sickness, including sickness from COVID-19, which can result in long-term impact and even death. I further understand that in my travels to, and in-person attendance at, the 2022 National Arts Awards, I may come into contact with persons who are not vaccinated against COVID-19 or other disease, or who have been infected with any such disease. By submitting an RSVP to attend in-person at the 2022 National Arts Awards, I knowingly and voluntarily assume these risks.

I agree to take necessary precautions while at the event including, but not limited to, engaging in appropriate social distancing, minimizing face touching, washing hands frequently, wearing a mask while inside Guastavino’s while not eating or drinking, and avoiding high risk environments as defined by the CDC.

I agree not to attend in-person any Americans for the Arts or affiliated event if I feel ill; have had recent exposure to another person with COVID-19; or have tested positive for, or been diagnosed as being infected with, COVID-19 within the 10 days prior to the 2022 National Arts Awards.

I agree to abide by all Health & Safety measures and requirements mandated by Americans for the Arts, Guastavino’s (event venue), the CDC, local, or federal government as it pertains to in-person gatherings.

I understand that non-compliance could result in denied entrance to or expulsion from the event.

By checking the waiver box on the registration form I acknowledge, and I agree that I have read and understood this waiver and will be bound by its terms and conditions. By checking the box on the registration form, I am waiving any right that I or my family, representatives, heirs, or successors might have to bring legal action or assert a claim against Americans for the Arts for any injury or illness sustained in connection with the 2022 National Arts Awards.
